How to Get Started

1. Complete our online application (see the tabs above this text).

2. Once submitted, we’ll send you the required FHSAA paperwork—including the EL2, EL3, EL7, and EL7V forms.

3. Return the completed forms to our school nurse before participating in your selected sport.

Program Costs

Application Fee: $75
Enrollment Deposit: $100
Athletic Fee: $200 per sport
Total: $375 for one sport (each additional sport is $200).

Contact Us

Florida High School Athletic Association (FHSAA), states that the student athlete "must undergo a pre-participation physical evaluation and be certified as being physically fit for participation in interscholastic athletics (form EL2). The physical is valid for 365 calendar days from the date that it was administered after which time the student must successfully undergo another physical evaluation to continue his/her participation. Parents and students must also submit a completed EL3 Consent and Release from Liability Certificate (5 pages). (FHSAA Bylaw 9.7)
